    Before beginning a supplement consult with Kate Mahoney, NTP, read the following information carefully, then sign and date.

    SUPPLEMENT CONSULTATION GOALS

    The fundamental goal of a Nutritional Therapy Practitioner, aka NTP, is to encourage people to become knowledgeable about—and responsible for—their own health. An NTP helps individuals reach their optimal level of overall health by supporting and bringing balance to the six foundations listed below.

      • Nutrient-dense foods
      • Hydration
      • Digestion
      • Blood Sugar Regulation
      • Fatty Acid Balance
      • Mineral Balance

    By supporting each of these foundations, the body's chemistry can be brought back into natural balance, setting the stage for optimal health. As a supplement expert, Kate Mahoney, NTP can provide information about supplement quality and the way specific products can provide nutritional support during the consultation.

    Nutritional Therapy is not designed to treat any specific disease or medical condition. As a Nutritional Therapy Practitioner, Kate Mahoney, NTP is trained to evaluate nutritional needs and make recommendations of dietary change and nutritional supplements, not medical diagnoses or prescriptions. No information, comment, or suggestion from Kate Mahoney, NTP should be construed as a medical diagnosis or prescription.

    Since every human being is unique, specific results cannot be guaranteed from how you, as a consumer, choose to use the information you learn. Therefore, as a consumer, you understand and accept that how you choose to use the information is your responsibility, and at no time in no way will you pursue legal action against the Academy of PSC or Kate Mahoney, NTP due to potential misuse or misunderstanding of the information provided during the consult.

    HEALTH CONCERNS

    If you suffer from a medical or pathological condition, it is your responsibility to seek medical attention with an appropriate healthcare provider. An NTP is not a substitute for a licensed general practitioner or other appropriate healthcare provider. A Nutritional Therapy Practitioner is not trained nor licensed to diagnose or treat pathological conditions, illnesses, injuries, diseases, or prescribe medications.

    If you are under the care of another healthcare provider and choose to take nutritional supplements, it is your responsibility to contact your other healthcare providers to inform them of your choice to use nutritional supplements. Supplements may be a beneficial adjunct to more traditional care, and because it may alter an individual’s need for medication, it is your responsibility to keep your general practitioner informed of changes you choose to make.

    If you are using medications of any kind, it is your responsibility to discuss any potential interactions between medications and nutritional products with your pharmacist. If you have any physical or emotional reaction to nutritional supplements you choose to take, it is your responsibility to discontinue supplement use immediately. If this occurs, it is possible the reaction may be an indication of the natural course of the body's adjustment. If you choose to do so, you may schedule another consult with Kate Mahoney, NTP to discuss this possibility.

  • COMMUNICATION

    Note that a Nutritional Therapy Practitioner does not diagnose or treat disease. Instead, Kate Mahoney, NTP will be providing supplement information during the consult regarding supplement quality and what nutrients could potentially help balance the body and promote optimal wellness. Any information provided by Kate Mahoney, NTP during the consultation is merely informational and is not medical advice.

    Every consumer is a biochemical individual, and it is not possible to determine in advance how your body will react to nutrients or supplements. It is your responsibility as a consumer to make a well-informed choice regarding any supplements you choose to take. Additionally, if you are on any prescription medications, it is your responsibility to speak with your pharmacist to find out about any potential contraindications before starting any supplement products.

    If your other healthcare providers have any questions regarding Nutritional Therapy, they are free to contact Kate Mahoney, NTP.

    LICENSURE

    NTPs receive a certificate of completion, similar to a degree, upon successful completion of all written and practical examinations. Kate Mahoney, NTP is also Board Certified in Holistic Nutrition by the National Association of Nutrition Professionals. She is not licensed by any state.

    CLIENT PRIVACY POLICY

    During the consultation, Kate may ask you to provide relevant personal details and information relating to your background, health, lifestyle, etc. (hereafter referred to as “Information”), including but not limited to:

      • Your full name, email address, phone number, physical address, date of birth, etc.
      • Your health history, including injuries, surgeries, prescriptions, etc.

    This Information will be gathered from you when you schedule your consultation and during the consultation, and used to:

      • Comply with all legal obligations Kate Mahoney, NTP has as a practitioner
      • Provide relevant information about supplements to support your inquiries
    To ensure the maximum benefit of Nutritional Therapy, it is important that any Information you provide is accurate and up-to-date. Additionally, if you start any new prescription medications, it is your responsibility to speak with your pharmacist to find out about any potential contraindications before continuing any supplement products. Like all practitioners, Kate Mahoney, NTP retains and maintains records of her consultations. It is your right as a consumer to access or update your records at any time. To do so, simply notify Kate in writing. There is a $20 administrative fee for providing a copy of these records.
  • Though NTPs are not HIPAA regulated entities, Kate Mahoney, NTP upholds the privacy best practices and the policies laid out in the U.S. Standards for Privacy of Individually Identifiable Health Information. She will take all reasonable steps to protect your Information from unauthorized access, use, or disclosure by using strong passwords, up-to-date software on all devices, and locking file cabinets for physical documents. However, even the best security practices cannot guarantee that all stored data will be completely free from third-party interception or corruption.

    In accordance with Standards for Privacy of Individually Identifiable Health Information, your consent is required for your NTP to collect, use, and disclose your personal Information. By signing below, you acknowledge consent for your NTP to collect your Information.
